Instructions for Certification Worksheet
In order to certify your enrollment to the VA, we must have a completed Certification worksheet on file for each semester that you wish to use your VA benefits. If the classes you enroll in are different than the classes on the signed Certification worksheet that has been turned in to the Veterans Services office, a new worksheet with the changes must be submitted. Failure to do so could decrease the number of credit hours certified to the VA.
There are two separate worksheets. One worksheet is for undergraduate and one worksheet for graduate students.
Instructions for completing the Undergraduate Certification Worksheet:
The undergraduate form has two pages. The first box must be completed by the student. The second box pertains to the courses that are being taken during the upcoming semester at the University of Kansas and must be signed by an academic advisor. The third box (on second page) should only be completed if the student is concurrently taking classes at another institution. This box also requires the advisor's signature. The student must sign and date the bottom of page two.

The graduate form has only one page. The first box and second box must be completed by the student. The third box pertains to courses that are to be taken during the upcoming semester at the University of Kansas and must be completed and signed by an academic advisor. The student must sign and date the bottom of the page.
