Kansas Board of Regents policy requires faculty and unclassified staff of Regents institutions to file a conflict of interest report upon employment and at least annually thereafter. In addition, federal policies require assurances that all investigators on a research project are in compliance with institutional conflict of interest reporting procedures at the time a proposal is submitted.

• Web-facilitated COI reporting
  COI reports can be initiated online and printed out for required supervisor signatures. A KU Online ID  is required to access the online system. Once the fully executed paper original is on file with Research Integrity, an individual is eligible to satisfy the annual reporting obligation by confirming, in just a few clicks online, that the most recent full report filed is still valid and up to date. Employees can confirm the most recent full report year after year, unless they have changes in circumstances to disclose. • COI forms and instructions for those who cannot file online
  All employees are encouraged to use the web-facilitated reporting system described above.  In case this is not feasible, employees may download the forms in MS Word or Adobe pdf format.  The option to confirm no changes described above is not available for downloaded forms.

Please note: The KU Conflict of Interest reporting obligation is separate and distinct from the requirement to file a State of Kansas Statement of Substantial Interests (KS SSI) with the Kansas Secretary of State.
