Step 1: Conduct an Inventory

a) Identify the necessary stakeholders to the process. These stakeholders should include:

b) Transportation Provider County Inventory

1. Send the Transportation Provider County Inventory to each transportation provider in your district.

2. Once you have the completed inventories from your providers, consolidate the information for each county in your district.

3. KUTC will provide assistance in calculating the demand based on the data in your inventory.

 

c) Human Service Provider Inventory

1. Send the Human Service Provider Inventory and Human Service Provider County Inventory to each human services provider in your district.

Step 2: Assessing Transportation Needs

a) KUTC will send you a report that provides your demand estimation for program and non-program transportation. Program related trips are those that would not occur but for the existence and operation of a specific social service program. Analyze the data and determine whether there are gaps or duplications.

Step 3: Identify Strategies to Address Gaps

b) After you have reviewed the results and inventory during your first meeting, each participant must complete the Framework for Action: Self Assessment Tool for Communities on their own.

c) Discuss the self-assessment as a large group. On a flip chart, capture the answers to the following questions:

Step 4: Develop Coordination Actions

a) Create a short list of priorities:

Consult the Framework for Action: Facilitator’s Guide pages 20-22 for tools to narrow many priorities into a short list.

Final Step: Implementing the Plan

This step requires the CTD and its implementation group to monitor the progress of the Coordinated Plan.

Each quarter the CTD will document the progress that has been made and what has been achieved.
