If evidence of pre-existing adequate coverage is provided before the insurance deadline, Feburary 1st, 2010(Spring 2010), a waiver will be granted and the insurance charge will be removed. The pre-existing coverage must be valid at least from the first day of classes through the last day of December for the fall semester or through the last day of May for the spring semester.
Students may obtain a waiver by presenting a signed and completed waiver form verifyling that the minimum coverages are met (see process below). The completed and signed waiver form must be received by ISSS at least 48 hours prior to the tuition due date, if you wish to pay the most accurate amount. The waiver form to be used for this certification can be obtained online or from the ISSS office. Once a waiver is approved, the insurance charge will be removed from the student's online account. The waiver process will differ depending on the insurance plan you hold and on your current status. For the procedures you need to follow, please view the following sections.
Alternate Coverage - Students with pre-existing insuranceIf you are not purchasing the KU insurance-UHCSR plan, you must complete and submit a waiver form along with a copy of your current policy's benefits summary page (in English) to waive coverage EACH SEMESTER you are enrolled. In order to apply for the waiver, please follow these procedures:
- Pick up a waiver form in the ISSS office or Download (Spring 2009) it now.
- Complete ALL fields in the top part of the waiver with your personal contact information.
- Complete the name of insurance, policy number, and coverage effective dates.
- Attach a copy of your policy's benefits summary page with the waiver form.
Sponsored Students
If you are a sponsored student receiving medical coverage through your sponsor:
- Pick up a waiver form in the ISSS office or Download Sponsored Student waiver now.
- Complete ALL fields of the form.
- Complete the name of your sponsor.
Attach a photocopy of your insurance card so that we can verify your coverage. If you do not have an insurance card, we will need a letter from your sponsor stating that you indeed have medical coverage. Failing to do so will result in a denial of waiver. It is important that you complete ALL the fields requested above so that we can expedite the waiver process.
Work Visa, Permanent Resident or U.S. CitizenIf you are a permanent resident or U.S. citizen, or currently hold a work visa (H, TN, L) through a company other than The University of Kansas, you are not required to show proof of insurance, and you do not need to complete a waiver form. Please ask ISSS to remove your insurance charge after verifying your immigration status.
