If evidence of pre-existing adequate coverage is provided before the insurance deadline: August 31 st (Fall), January 31 st (Spring)
a waiver will be granted and the insurance indicator will be removed. The pre-existing coverage must be valid at least from the first day of classes through the last day of December for the fall semester or through the last day of May for the spring semester.
Students may obtain a waiver by presenting a signed and completed waiver form verifyling that the minimum coverages are met (see process below). The completed and signed waiver form must be received by ISSS at least 48 hours prior to the insurance deadline. The waiver form to be used for this certification can be obtained online or from the ISSS office. Once a waiver is approved, the insurance indicator will be removed from the student's online account. The State of Kansas offers an insurance benfit to qualifying GTAs and GRAs. Please contact Mary Karten, Staff Benefits, 135 Carruth-O’Leary Hall, 785-864-7346, for information regarding this option. ISSS will periodically verify the insurance enrollment with Staff Benefits and update your account accordingly.
NOTE: IF YOU PURCHASE THE GTA/GRA INSURANCE, YOU DO NOT NEED TO COMPLETE A WAIVER FORM.

Attach a photocopy of your insurance card so that we can verify your coverage. If you do not have an insurance card, we will need a letter from your sponsor stating that you indeed have medical coverage. Failing to do so will result in a denial of waiver. It is important that you complete ALL the fields requested above so that we can expedite the waiver process.
