Go the following website: https://www.uhcsr.com/selfservicesupport/students/collegeStudents.aspx

Step 1 - Select School or Association

1. Select School.
2. Go to "Find my School's Plan".
3. Select Univ. of Kansas in the next drop down box.
4. Click "Go".
5. On the left hand side of the screen, select "Enroll Online Now".

Step 2 - Select Coverage and Rates

1. Enter date of birth using mm/dd/yyyy format.
2. Answer Yes or No for including the spouse in the policy.
3. Answer the number of children to include in policy.
4. In the box that requests your permanent address zip code, please put your local U.S. zip code (mailing address).
5. Click continue.
6. Scroll down to International Student Plan.
7. Choose option the appropriate option (This is the amount you will be responsible to pay when completing the enrollment form.)
8. Press Continue.

Step 3 - Create Your Account

1. Fill your first name.
2. Fill in your last name.
3. Fill in your email address.
4. Confirm your email address.
5. Choose a username and password, and then confirm the password.
6. Click continue.

Step 4 - Provide Student Information

1. Fill in your middle initial.
2. Check male or female box.
3. Provide expected graduation date.
4. Provide phone number.
5. Provide social security number or student ID#.
6. In the permanent address box, please put your U.S. mailing address; DO NOT put your overseas address.
7. Check the box stating that your permanent address is the same as your mailing address.
8. Click continue.

Step 5 - Enter Payment Information

1. Choose whether payment is using a credit card (MasterCard or Visa) or an electronic check.

Step 6 - Verify Payment Information

1. Make sure all of your information was entered correctly.

Step 7 - Confirmation

1. Print a copy of the online confirmation page
2. Print a copy of your temporary insurance card

INSURANCE DEADLINE FOR SPRING 2008 IS JANUARY 31st!
