A complete audiological evaluation completed within the last three years or
the last evaluation with a statement by the certifying professional that the
hearing loss is/has been static since that time.
Please include the following:
aided and unaided pure tone thresholds
speech recognition thresholds
suprathreshold speech tests (i.e. word recognition scores and at what levels)
tympanograms
if hearing aids are used, their compatibility with infrared or FM Assistive
