Self-nomination Applications:
Office of the Provost - Friday, September 14, 2007

Endorsed Applications:
The College Office - no later than 5:00 p.m. on September 21, 2007
200 Strong Hall

All applications reviewed by the College Committee on Sabbatical Leaves will be submitted to the Office of the Provost by:
November 1, 2007

Preparation of Files for the College Office: 1 letter-sized file folder, labeled with Name, Department, and Original • 1 original application
1 letter-sized file folder, labeled with Name, Department, and Copy • 1 photo copy of application
A 1" expandable file folder labeled with Name and Department • Place both letter-sized file folders in expandable folder
