The ARTS Form Request Page is a service that is not intended to replace your academic advisor. If you have any questions about your ARTS form, please contact your school.

KU Lawrence/Edwards Campus Students, in order to request or retrieve an ARTS form:

• You must be currently enrolled at KU
• You must have a KU Online ID

KUMC Students, in order to request or retrieve an ARTS form:

• You must be currently enrolled at KUMC
• You must have a Novell login

You will only receive the ARTS form(s) for the school(s) in which you are enrolled.
